In an application where a USB hub is an add-on card, how can I terminate the upstream port if disabled?

We don't recommend disabling the upstream port. USB is a host-centric bus and disabling the upstream port would break that connection to the host, rendering the hub useless. You can power down the USB hub in this instance, or just hold the USB hub in RESET.
